Tienes derecho a:<\/b><\/span><\/h3>\n

  1.  Espere ser tratado de manera justa, cort\u00e9s y con respeto en todo momento.<\/li>\n
  2.  Recibir servicios sin discriminaci\u00f3n por motivos de raza, edad, sexo, credo, origen \u00e9tnico, religi\u00f3n, orientaci\u00f3n sexual, discapacidad, estado del VIH, fuente de pago o creencias pol\u00edticas. (Si es menor de 18 a\u00f1os, algunos servicios pueden requerir el permiso de sus padres).<\/li>\n
  3.  Recibir atenci\u00f3n y servicios que cumplan con la Ley de Estadounidenses con Discapacidades de 1990. Los servicios se brindar\u00e1n en el entorno menos restrictivo disponible.<\/li>\n
  4.  Conozca el nombre y los antecedentes profesionales de cualquier persona que trabaje con usted.<\/li>\n

    1.  Tiene la responsabilidad de informar al programa de Servicio de Salud de Shawnee o al Centro de Salud correspondiente si no puede asistir a una cita.<\/li>\n
    2.  Tiene la responsabilidad de proporcionar, a su leal saber y entender, informaci\u00f3n precisa sobre quejas actuales, enfermedades pasadas, hospitalizaciones, medicamentos y otros asuntos pertinentes relacionados con el plan de atenci\u00f3n.<\/li>\n
    3.  Tiene la responsabilidad de informarnos si siente que lo tratan injustamente.<\/li>\n
    4.  Tiene la responsabilidad de hacer preguntas hasta que comprenda claramente el curso de acci\u00f3n propuesto y lo que se espera de usted.<\/li>\n
    5.  Tiene la responsabilidad de cumplir con el plan de atenci\u00f3n recomendado por el profesional de la salud.<\/li>\n
    6.  Tiene la responsabilidad de comprender que el rechazo del tratamiento o el incumplimiento de las instrucciones del profesional de Shawnee Health Service podr\u00eda resultar en resultados indeseables, por los cuales el paciente y \/ o la familia aceptan la responsabilidad.<\/li>\n
    7.  Tiene la responsabilidad de participar activamente en su propia atenci\u00f3n m\u00e9dica.<\/li>\n
    8.  Tiene la responsabilidad de mantener una cuenta corriente si los servicios requieren pago. El pago vence al momento del servicio o dentro de los 30 d\u00edas posteriores a la fecha del servicio. Hay planes de pago y un plan de descuento disponibles para pacientes elegibles.<\/li>\n<\/ol>\n
