As Your Health Home, your continuum of care is important to us. Transitional Care Management (TCM) helps patients during their transition from hospital to home.
Our goal is to improve patient care and reduce mistakes in care coordination that may lead to hospital readmission.
If you choose to participate in Transitional Care Management, a Shawnee Alliance care manager will contact you within 48 hours of discharge from the hospital to schedule a follow-up appointment.
Then our care managers will ensure that you have made any necessary follow-up medical appointments. We will also see to it that appropriate care is set up in your home and your prescriptions are filled. Most of all we want to be sure that you have all the services necessary to recover and remain in your home.
